Stage One Appeal Form

In order to submit an appeal, you must meet one of the three grounds found in Section 10, Chapter 3, Para 2:

a. there is reasonable ground supported by authoritative and objective evidence to believe that there has been administrative or procedural error of such a nature as to have affected the outcome of the investigation or result appealed

b. the decision in the case was manifestly unreasonable

c. there is new evidence that for good reason, objectively and authoritatively documented, could not be submitted earlier

Section 10, Chapter 3, Para 5 provides that appeals must be submitted within 10 working days of the letter containing the decision you wish to challenge. If 10 working days have lapsed, you must further explain and evidence why you were unable to submit your appeal within the prescribed time limit.
